PUBLIC OFFER AGREEMENT
for the provision of psychological services

According to Art. 631, 634 of the Civil Code

1. Parties to the agreement
The client is an adult capable person who is sober during the session.
Psychologist – Kravchenko T. V. (FOP Kravchenko T. V., TIN 3021700712, Dnipro, Rubinovy ​​Boulevard 5/259).

2. Subject of the agreement
The psychologist provides psychological counseling / psychotherapy services.

3. Conclusion and validity of the agreement
The agreement is considered concluded from the moment of payment for the first consultation and is valid until the parties’ agreements are fulfilled.

4. Cost and payment
The cost is determined in private correspondence (Viber, Telegram, Instagram, WhatsApp, etc.).
Payment is made in advance via WayForPay (Visa and MasterCard) or to the bank details of the individual entrepreneur.

5. Registration and duration
A consultation is booked only after its prepayment.
Session duration – 60 minutes. The Client’s lateness does not extend the session time. The Psychologist’s lateness – extends the session time by the time of the delay.
The Client’s lateness for more than 30 minutes is equal to the consultation.

6. Cancellation and refund
Free cancellation or postponement of the session by the Client is possible no later than 24 hours before the session.
If canceled later – the payment is not refunded.
If the Psychologist cancels the session less than 24 hours in advance, the Client is refunded and a free session is provided.

If the Client or the Psychologist cancels in advance, the funds are refunded or the consultation is postponed by agreement.
Refund options:
– Payment to a payment card to the Client’s current account
– Payment to the Client’s bank account details

7. Confidentiality
Both parties are obliged to maintain complete confidentiality.
The Client grants permission to process and store his/her personal data (in accordance with the Law of Ukraine “On Personal Data Protection” No. 2297-VI).
The Psychologist may use cases in supervision without disclosing the Client’s identity.

8. Information interaction
Changes in the cost of services are notified before booking the next consultation.
Outside the consultation, only organizational issues (registration, payment, transfer) are discussed. Outside the consultation, the Psychologist does not consult the Client. Everything that concerns the Client is discussed during the next scheduled consultation.

9. Relationship boundaries
During the term of the contract, the parties do not enter into any other relationships (except educational and therapeutic) and do not add friends on social networks in order to maintain professional boundaries.

10. Mental state
A client with diagnosed psychiatric disorders is obliged to inform the Psychologist about this before the first meeting to assess the specialist’s competence. Otherwise, the effectiveness of psychotherapy may be lost.
