Contract-offer for the provision of psychological services (edition dated 04.26.2024)

The contract is concluded between Psychologist Kravchenko Taras and the recipient of psychological services on the terms of the offer.

1. Participants in the contract:
– individuals, adults who are not in a state of alcohol or drug intoxication. (Hereinafter referred to as the “Client”)
– Psychologist Kravchenko TV (hereinafter “Psychologist”)

2. The subject of the contract – psychological counseling / psychotherapy.

3. The contract is considered concluded from the moment of full payment by the Client for the first consultation and is valid until all obligations are fulfilled.

4. Prices for the Psychologist’s services are stipulated in the personal correspondence between the Psychologist and the Client (Viber, Telegram, Instagram, WhatsApp, Messenger FB, e-mail and other means of communication).

5. Payment for services is made by advance transfer using the payment option chosen by the Client.
Payment options:
– Payment by Visa and MasterCard payment cards – WayForPay
– Payment to the bank account details of the IE Kravchenko T.V.

6. Reservations for consultations are made by prior appointment and only after payment of the full amount of the cost of the service. Otherwise, the consultation time is not reserved.

7. Free cancellation or rescheduling of a booked consultation is possible on the day preceding the day of the consultation.
Cancellation or rescheduling of the consultation by the Client on the day of the consultation (without good valid reasons) is paid in the amount of 100% of the cost of the consultation, that is, the prepayment amount is not refundable.

8. Duration of consultation – 60 minutes. The time is counted from the scheduled start of the consultation. If the Client is late, the consultation time is not increased. If the Client is more than 30 minutes late, the consultation is considered completed and is paid for in accordance with Clause 7 of this contract-offer.

9. If the Psychologist fails to appear for a consultation without notifying the Client on the previous day or without a good good reason, the Client receives a full refund of the prepayment.

10. In case of non-appearance of the Client for the scheduled consultation with a warning or subsequent notification to the Psychologist about the existence of good and valid reasons for non-appearance, or in case of non-appearance of the Psychologist for the scheduled consultation with a warning or subsequent notification to the Client about the existence of valid and valid reasons for non-appearance, the Psychologist returns the money paid by the Client in the form of an advance payment to the bank details provided by the Client in personal correspondence with the Psychologist.
Refund options:
– Payment by payment card to the Client’s current account
– Payment to the bank details of the Client’s current account

11. The duration of psychotherapy is determined by the Client.

12. The psychologist strictly keeps confidential personal and other data of the client.

13. Rule for all types of counseling: at the initial appointment to the Psychologist for a consultation and non-attendance without warning – re-registration is not carried out.

14. The Psychologist informs the Client about the change of prices for his services at the end of the consultation, paid at the previous cost.
15. If the Client has psychiatric diseases diagnosed by a psychiatrist, he must inform the Psychologist about this before booking the first appointment, so that the Psychologist has the opportunity to determine his own competence in working with such a Client. Otherwise, the effectiveness of psychotherapy may be lost.
